I love it! Your display name must be your common name, but not a nickname; so if your wallet-name is Charles but people call you "Chuck", "Chuck" can be your display name. Even though it's a nickname, so it should be in Other Names. Apparently they can't be consistent for three sentences at a time...
I put the name that I am commonly known by and my pseudonym in both those fields (they are the same name btw). At least 'I' am consistent. :)

Ah, that's an easy one. Remember they said they were going to warn people before suspending them at some point in the future?I believe that time is now, and you're being warned that unless you sort things to Google's satisfaction that they'll suspend you again.
Tateru: That's certainly possible, but the timing didn't make a lot of sense in that case. But since not very much related to their handling of this situation is making sense, there's no telling!
